All learners accepted into a Per Scholas training are required to submit documentation verifying eligibility criteria for enrollment has successfully been met. Your documents must:
  • Be legible. We cannot accept damaged or illegible documents.
  • Have the same first and last name (If your name changed, upload a copy of your marriage certificate, divorce decree, or court issued name change. Documents must be issued in the U.S.). 
  • Cannot accept documents where you are not clearly recognizable. 
  • Your proofs of residence must show your current address (cannot be a P.O. Box). 

About this Document Verification Portal

Per Scholas recognizes that the efficient management of its electronic records is necessary to support the organization's core functions and to comply with its legal and regulatory obligations. Information collected and documents uploaded and sent through this portal applies a cryptographic function to data that results in ciphertext. Specifically, the encryption process uses symmetric key encryption and a 256-bit Advanced Encryption Standard (AES) algorithm using CBS mode, PKCS5 padding, and a randomized, 128-bit initialization vector (IV) to encrypt data and files sent through this platform and stored on Per Scholas Salesforce platform.  

Your email address must be the same one used when you first applied/submitted your application to Per Scholas
Note: The collective documents/attachments uploaded should not exceed 35MB. As a reference, each document uploaded should not exceed 4MB
Proof of Eligibility to Work in the U.S.

Employees must provide documentation to their employers to show their identity and authorization to work. To ensure you're set up for employment opportunities post-training, Per Scholas requires proof of I-9 documents. These document(s) must establish both Identity and Employment Authorization. Jobseekers have two options for proving their employment eligibility:

Option A: Upload 1 Document that Establishes Both

Option B: Upload 2 Documents - 1 that Establishes Identity + 1 that Establishes Employment Authorization


List A - Documents that Establish both identity and employment authorization or List B - Documents that establish identity plus List C- Documents that establish employment authorization
Please select which option you will be uploading for your verification. All documents must be UNEXPIRED at least 6 months after your graduation date. If your document is expired, you must submit an eligible receipt for extension (form I-797) in the misc section and notify your admissions lead.

Name Change Verification


You will need to upload certified copies of documents showing how your name changed (example: marriage certificate, divorce decree, court-issued documents, etc.). 

If your name has changed multiple times, you must bring multiple documents that link all names.
Additional Course-Specific Documentation (if applicable)
The course you are enrolling in may have additional documentation required as determined by licensing boards or funders. Here is where you should upload these documents if your admissions lead notified you of these additional required documents.
